We're upgrading how Forms work in PandaDoc. Instead of being a separate object, forms are now a step inside your template workflow — giving you more flexibility and a more streamlined document creation experience.
This article explains what's changing, what happens to your existing forms, and what you need to know before the migration is complete.
Heads up: The migration is rolling out gradually. You'll receive an email notification when your account is scheduled to migrate.
What's changing
Previously, Forms were a standalone object you created and managed from the Forms section in the left navigation. Going forward, forms live inside templates.
The new experience also brings some powerful improvements:
Fully customizable questions — you're no longer limited to role-based form fields. Add any question you need, and the answer is automatically mapped to a variable that you can place anywhere in your template content.
Dynamic document names — now you can use variables in your template name so every document generated from a form is named automatically based on the responses (for example, "NDA — Acme Corp" instead of a generic title).
Dropdown fields for standardized answers — turn any variable into a selection list so respondents pick from pre-defined options instead of typing freely, keeping your data consistent.

What happens to your existing forms
Your forms are migrated automatically. However, if you use Zapier with your forms, action will be required — see Zapier integrations below. Migration is being rolled out gradually, so different accounts will be migrated at different times. You'll be notified before your account is migrated.
Each existing form is copied into a connected template. Your form's content, fields, and settings move with it. Here's what you'll see in the product after migration:
The Forms section remains in the left navigation, and your existing forms will still appear there. Note that this section will be removed in a future update — you'll be notified when that happens.
A banner at the top of the Forms list will inform you that forms now live inside templates.
When you click on an existing form, a pop-up will appear explaining that the form has moved to its connected template, with a Go to template button to take you directly there.
From there select Share as public form button at the top right to copy the link to the form or the embed code.
Creating new forms from the Forms section will no longer be available. To create a new form, open a template, add a workflow step, and configure it as a form.
Note: Your existing public links and embed code will continue to work. Recipients who access a previously shared link or embedded form will be redirected to the new form experience automatically.
⚠️ Important: Zapier integrations will need to be reconfigured
If you have Zaps set up in Zapier that use your existing forms as a trigger or action, those Zaps will stop working after your account is migrated.
This is because your forms are moving into templates, which are treated as different objects by the Zapier integration. Your existing Zaps won't automatically reconnect to the new template-based forms.
What you need to do:
Before your migration date, make a note of all Zaps connected to your PandaDoc forms.
After migration, open each affected Zap in Zapier and reconfigure it to use the corresponding template (the one your form was migrated into).
Test each Zap to confirm it's working correctly before reactivating it.
You can find your migrated form's connected template by going to the Forms section, clicking the form name, and selecting Go to template in the pop-up.
Where to find your form responses
Responses submitted through your forms are stored as documents. You can find all documents generated from a form in the Documents tab, inside a folder named after the template.
What's different in the new experience
Most core functionality carries over, but there are some differences to be aware of.
⚠️ Important change to multi-role forms: In the old experience, each recipient filled out their own part of the form — the primary recipient entered their details, then secondary recipients were prompted to enter theirs separately. In the new experience, one person fills out the entire form. If your form has multiple roles (e.g. a sender and a client), the person filling out the form is responsible for entering the contact details for all roles before the document is generated. There is no longer a separate step for secondary recipients.

Frequently asked questions
Will my embedded forms break?
No. Your existing embed code will continue to work. Recipients will see the new form experience, but the link and embed will function as before.
Do the new forms use usage credits?
Yes. Each document generated from a form consumes one usage credit, the same as before.
Can I still use the Forms section after migration?
Yes, the Forms section remains in the navigation and you can view your existing forms and their response counts there. However, you'll no longer be able to create new forms from this section — form creation now happens inside templates.
What if I relied on CSV exports or pre-fill?
These features are coming later in Q2. If your workflow depends on them, your existing forms will remain accessible until your account is migrated, and your submitted responses are not affected.
Is there any plan or pricing change?
No. Forms remain available on Business and Enterprise plans, with the same usage credit structure.
How do I get the link to my form from the template?
When you open your old form, select Go to template button in the pop-up.
When the template opens, select Share as public form button at the top right to copy the link to the form or the embed code.
