How do I put radio buttons on my document?
Radio buttons allow your document’s recipients to select only one predefined option from the list you provide.
To add a radio button field, select the field on the content panel, then drag and drop it to where you’d like to place it in your document. Two options will be added to the radio button field by default.

What if a Radio Button isn't right for you or your template design?
Workaround #1. The drop-down field:
Workaround #2. The other one is a bit more sophisticated and allows you to make an actual nice-looking radio button list.
a) Put a pricing table block on your document/template and add a section:
b) Hit the down-arrow button on the right -> Recipient Options -> Enable Multiple Choice:
с) Add a few rows under the pricing table section:
d) Now, you might want to change the section name and type the options:
e) Let’s delete the first row and hide columns we no longer need:
f) Finally, change the header background color and remove table borders:
